While the concept of a viable and nonviable pregnancy is relatively easy to grasp, it is governed by strict definitions. From a clinical perspective, a viable pregnancy is one in which the baby can be born and have a reasonable chance of survival. By contrast, a nonviable pregnancy is one in which the fetus or baby has no chance of being born alive. The definitions are designed to prevent the termination of a pregnancy if, in fact, there are any reasonable measures to ensure the baby’s survival. Within this broad definition, the one word that is open to interpretation, of course, is “reasonable. Is the definition fixed, or can it vary by doctor, hospital, stage of pregnancy, or even income? Experts have aimed to provide clarity, not only from an ethical and legal standpoint but to offer parents the assurance that they’ve made the right choice based on the weight of current medical evidence. From a diagnostic perspective, nonviable does not mean a little chance of survival.

A Fetal Viability Scan is an ultrasound examination, usually carried out at 7 to 12 weeks , to confirm that the pregnancy is developing normally and to provide reassurance. Additionally, this scan will confirm or exclude whether twins are present.

A dating scan is an ultrasound scan to determine how many weeks pregnant you are and your due date. Read on for more information about what a dating scan offers. The sonographer will put gel on your abdomen and move a hand-held device called a transducer, back and forward over your skin. In some situations, such as when the scan is completed before 10 weeks of gestation or you are overweight, you may also need a vaginal scan.

Your sonographer will let you know if this is needed.

First-trimester ultrasound

Due to the size of the developing structures and embryo, it is uncommon to perform an ultrasound before 5 weeks gestation 5 weeks after the start of your last menstrual period. Ultrasound is not consistently able to confirm viability and fetal heart activity prior to 6 weeks gestation, so unless there is an urgent need to have an early pregnancy and dating scan earlier most of these ultrasounds are performed after 6 weeks and ideally at 8 weeks for dating purposes.

Urgent cases may still be performed earlier, please discuss this with your referring doctor. Your ultrasound may be performed transabdominally or transvaginally. Transvaginal ultrasound involves placing a thin transducer slightly thicker than a tampon into the vagina.
